Holiday rotation.The Registered Nurse (RN) Coordinator position handles the day-to-day coordination of the team clinical functions, assists in implementing the patient plan of care with staff, volunteers, and outside agencies. Communicates as a liaison between staff, patients, and outside agencies. Assists with maintaining records. May involve direct patient care.Essential Duties and Responsibilities for the Registered Nurse (RN) Coordinator:Assists in coordination and implementation of the patient plan of care.Handles phone calls from the patient, primary care giver, or designee regarding any questions or concerns.Collaborates with the interdisciplinary teams, RNs, or Team Manager.Coordinates and schedules patient visits.Provide timely documentation/data entry per Hospice time frames and deadlines.Communicates current health status and needs of patients to appropriate team professionals.Assists the team and Team Manager with other administrative functions, as necessary.May occasionally be called on to make field visitsMay periodically perform the basic Charge RN functions when needed to cover the designated Charge RN.Qualifications for the Registered Nurse (RN) Coordinator:Education/CertificationGraduate of an accredited School of Registered Professional Nursing with a current RN licensure in the State of New York.Experience1-2 years of nursing practice as a Registered Professional Nurse preferred.
