Graduate of an accredited school of nursing required; BSN preferred.Experience:Minimum three years' experience in home care or hospice, required.Previous experience in managed care, infusion care, and case management and previous home health/hospice case management experience; preferred.Licensure/Certification: Current RN licensure in appropriate state, required.Additional Skills Required:Excellent nursing assessment skills/judgment and understanding of Hospice philosophy.Knowledge of Hospice services necessary to obtain and interpret information in terms of the patient's needs in order to appropriately receive referrals.Knowledge of Medicare/Medicaid rules/regulations necessary to understand criteria for determining patient eligibility for admission.Knowledge of nursing and pharmacy procedures and standards of practice, both general and specific, to the specialties of infusion and enteral therapies.Strong writing skills, organizational and analytical skills.Must be flexible, adaptable to change and able to apply problem-solving skills and critical thinking skills.Ability to interact effectively with diverse individuals at all levels of authority, in teams and on committees.Must have computer experience and have reliable transportation to and from all work related assignments and activities.Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair.Ability to work in environment containing distractions and interruptions such as noise and telephone calls.Ability to handle varying and unpredictable situation such as emergencies or crisis situations.Must be available for after hours call on rotational basis.Specific vision abilities required by this job include close vision, distance vision, night driving, and ability to adjust and focus.Job Opening ID21684
