Enjoy a great work culture, unique work options and schedules, while building heartfelt relationships with those needing personalized care.To provide all aspects of skilled nursing care and management of patients who are terminally ill or have a life-threatening disease. Supervises the aide(s) and Hospice Patient/Family Support Volunteer. Actively provides supervision of LPNs as it relates to patient care. Reports admission information and changes in the patient's condition to the Patient Care Coordinator (PCC) and implements changes to the plan of care as discussed. Provides timely and appropriate patient/family education and support based on diagnosis and expected outcomes.Identifies need for other internal and external patient care services, makes appropriate referrals, and coordinates the overall care provided to the patient. Participates in quality assessment and improvement activities. Consistently provides responsible, professional guidance and support to the LPNs to assure continued clinical growth and excellence. Presents positive attitude through change, supports all organizational activities and processes directly benefitting the entire VNA Family of Services. Actively supports team members in every aspect of delivering care and daily responsibilities.Provides timely and appropriate patient/family education and support based on diagnosis and expected outcomes; which prepare them for palliative care and symptom management at the end of life.Teaches patients/families skills for independent functioning, improvement of patient comfort and quality of life, and health promotion.Provides 'Caring Decisions' as necessary.Incorporates safety issues into plan of care.Establishes scheduling patients based on patient/family need, managed care criteria, projected positive outcomes, judicious management of health care benefits, and productivity expectations.Has a clear understanding of the organization's mission, vision and policies.Utilizes strong, consistent communication skills to provide holistic patient care, patient/family satisfaction, LPN supervision, mentoring, efficient scheduling and physician/facility satisfaction.Complies with safety policies and procedures at all times and informs the PCC or Manager of any unsafe conditions.Identifies need for other internal and external patient care services, makes appropriate referrals, and coordinates the overall care provided to the patient.Communicates in a clear concise manner, with the recipient's perspective in mind.Presents positive attitude through change, supports all organizational activities and processes directly benefitting the entire VNA Family of Services.Indentifies actual and potentially unsafe conditions for patients/families whether emotional, physical, psychosocial, or environmental in nature and takes corrective action.Assists and supports family through physical/emotional crises, changes in level of care, decision making and impending death.Actively supports team members in every aspect of delivering care and daily responsibilities.Follows the organization's policy regarding the confidential treatment of patient (including HIPAA), employee, and proprietary information.Documentation reflects coordination of care between patient/family, MD, and other team members.Offer recommendations to streamline documentation and improve communication.Seeks ongoing education to advance clinical expertise.Demonstrates self-direction to best utilize available resources and ability to meet the multi-faceted demands of providing care in home care, managed care, and hospice benefit arenas.Recognizes the importance of empowerment in problem-solving and decision making.Supports, coaches, and mentors team members to assure the provision of excellent patient care and strong, effective communication.Uses technology provided as intended and within established guidelines.Maintains the judicious use of the organization's equipment, materials, and supplies to meet patient needs and reports malfunctions or defects.Documents the utilization of equipment timely and appropriately.Handles all problems and concerns with the highest level of professionalism, tact and respect.Discuss appropriate customer responses with the Manager to ensure a proactive approach to working with our clients concerns.Acts as a liaison to resolve all customer concerns and/or problems between the organization and clients.Greets customers (external & internal) with a smile.
