Job Description:
*JOB SUMMARY*Responsible for the oversight of the medical and clinical condition of the consumers. Interacts with and works closely with the consumers physicians and medical specialists, to promote consumers' medical well being. *OPWDD experience required.**DUTIES AND RESPONSIBILITIES** Responsible for Medication Administration by certified, direct care staff:* Teaches the New York state approved course, and keeps records for each staff who become certified and ensure staff are re-certified prior to due date. OPWDD guidelines* Review and ensure the review of MAR and medication packages weekly to ensure proper dispensing of medications and treatments.* Identify medication error and follows policy for counseling staff, filling out the appropriate forms, and notifying appropriate personnel in a timely manner.* Evaluate minor medical problems and implement proper treatment and follow up for all consumers.* Accept on-call responsibilities as required by the GCHCC, Inc. Policy:* During designated on-call hours, must remain available by telephone or cell phone to respond to emergency situations/staff medical concerns relating to consumer care.* Must complete all paperwork relating to emergency care provided, and notify others of actions taken as per policy and procedure.* Ensure monthly doctor orders are correct, complete, in MAR, and initialed.* Complete all paperwork required to implement medical/pharmacological orders received by MD's.* Ensure appropriate amount of medication and valid dates are available for each consumer.* Ensure paperwork is complete for transportation of consumer to medical appointment(s).* Maintain orderly records for each individual consumer in medical record book.* Provide training to consumer in self-administration of medication or other medical goal plans as indicated by the Individualized Service Plan.* Provide orientation and training to staff so that they may provide appropriate care to consumer.* Complete semi-annual nursing assessments.* Ensure that medical equipment is adequate and in good repair.* Attend annual reviews, special treatment meetings, and other meetings as requested.* Respects consumer rights at all times.* Follow safety and infection control procedures at all times.* Maintain consumer/facility confidentiality at all times.*Education:*Graduate of accredited school of nursing and RN license in NYS.*Training, License, Registration:*Valid NYS Driver License. NYS RN License. Maintain certification in the following areas: CPR, SCIP, Medication/Certification/vitals.*Experience:*One year experience with the *OPWDD* required.*Interpersonal Contacts:*Staff, consumer, family members, and various community service as needed.*Working Conditions:*Community residence, shared office, well lighted, some travel, exposure to bloodborne pathogens, chemical hazards. Exposure to inside and outside environmental conditions, flexibility to work weekends and on call.*Physical Demands:*Ability to balance, lift, pull, push, stand, stoop adequately to perform the duties and responsibilities of the job.
