Pay Range: $38.46 - $43.27 an hour.By adhering to Connecticut State Law, pay ranges are posted. The hours are Monday through Friday 8:30am to 5:00pm. The position is located primarily in Guilford but will travel to West Haven, New Haven, Milford, Wallingford and other locations as needed.Under immediate supervision, supports providers including physicians, nurse practitioners, and physician assistants in delivery of patient care.SummaryUnder immediate supervision, supports providers including physicians, nurse practitioners, and physician assistants in delivery of patient care. Essential FunctionsProvides support to assigned Provider, including medication refills, completion of patient forms, patient reminder calls, patient follow up on med changes, lab results, future appointment needs, etc., and liaison between provider and incoming callers outside agencies, providers, patients, etc.Assists with providing patient care such as records vital signs, discusses and records chief health concerns, performs tests, and administers injections.Assist with triage of patients walking into the clinic to be seen; this entails assessing the patient and determining the priority for which the patient needs to be seen and coordinating the appropriate care based on the assessment.Provide phone triage by collecting appropriate information over the telephone to determine the urgency of a health problem and to determine whether medical intervention is needed and how soon treatment should begin.Places phone calls to pharmacies to order prescriptionsSet-up referral contacts.Pulls and prepares patient chartsAnswers and follows up on patient inquiries. Maintains neatness of work areaDocuments all patient care and completes tasks in EHRSkills and KnowledgeDemonstrated knowledge and skill of nursing theory and practicesDemonstrated knowledge and skill of EHR and other software systemsStrong communicationEducation and ExperienceRN Degree from accredited schoolCurrent State RN license
