Vestal Park Rehabilitation and Nursing Center is a beautiful 160 bed skilled nursing facility located in Vestal, NY and are currently looking for an experienced (will be willing to train the right individual) and energetic RN to join our team as a Wound Care Nurse.
Responsibilities:
- Oversee the facility's wound prevention and management program
- Assess, measure and treat wounds as needed.
- Communicate changes in residents' conditions to the interdisciplinary care plan team and other partners as appropriate
- Attend Quality Assurance meeting and report on applicable information
- Attend and prepare for weekly Case Management meetings
- Ability to educate staff by conducting in-services and one on one learning
- Ability to investigate, track and trend identified areas of opportunities
- Develop action plans for areas of needed improvement
- Provide partner support and coaching
Requirements:
- Wound Care Certified (preferred)
- Current NYS RN license required
- Prior Wound Care experience preferred
- Excellent physical assessment, critical thinking and time management skills
- Ability to perform effectively in fast-paced, stressful environment
- Excellent communication skills
