Total Compensation Estimator ToolYour Specific Responsibilities:This position performs high level professional duties and responsibilities in the care and treatment of patients, at and for the Hernando County Health Department. Provides and maintains continuity of health care services to clients within the clinic setting. This position performs duties and responsibilities providing nursing services to clients being seen at the Department of Health in Hernando County in the Brooksville and Spring Hill Clinic(s), under the direction of the Senior Community Health Nursing SupervisorTakes and records preliminary medical history, vital signs and giving prescribed medications under protocol. Documents in the record, utilizing problem oriented Subjective, Objective, Assessment and Plan (S.O.A.P.) method.Provides nursing assessments and may make a nursing diagnosis and treat per protocol in the Family Planning and Sexually Transmitted Disease (STD) clinics, counseling and educating as indicated: explaining the disease, its complications and its prevention.Interviews clients for evaluation and assessment of patient's condition, counseling and treating conditions covered under Registered Nurse Standard of Care Protocols.Assess immunization records, administer vaccines to children and adults per protocol.Assists the physician and/or APRN with medical procedures per county health department procedures.Provides Pre/Post HIV (Human Immunodeficiency Virus) counseling to clients, including prevention with the educational component.Teaches healthy lifestyles, family planning methods, and risk reduction in the clinic and community setting.Advises clients of results and counsels on preventative measures and laboratory results per the procedure or protocols of the unit.Provides education and counseling on side effects, palliative measures and follow up vaccines.This position has responsibilities to case manage for those client's needing treatment and follow-up for sexually transmitted diseases. These duties include providing education, scheduling appointments, providing nursing care and services, and follow up related to those needing additional treatment and to those who do not keep their appointment.Dispenses insulin, Epilepsy medications per private physician and/or protocols of Department of Health in Hernando County.Provides referral information to clients and other providers to facilitate continuity of health care. Responsible for providing and ensuring case management nursing services to clients.Monitors vaccines are stored at appropriate temperature, uploads vaccine log temperatures into FL Shots. Performs date entry of all immunizations given into FL Shots ensuring accuracy and billing information. Assists in reviewing and assessing records for proper documentation for Quality Improvement reviews.Maintains and updates professional knowledge by utilizing educational opportunities in workshops, seminars, in-service and participating in professional and related organizations.Responsible as a need-to-know employee for security sets relating to active medical records as it relates to performance of assigned duties and responsibilities for security and confidentiality of same.Responsible for securing the designated information set with the purpose of protecting confidentiality, data integrity, and appropriate access for all information, both confidential and public record, which is stored in hard copy or electric format.Performs other related duties as requested/required.
