Summary:

Coordinates telemonitoring systems and processes for Catholic Health Home Care to improve patient outcomes. Demonstrates exceptional triage skills and appropriate judgment/assessment skills to be able to make independent clinical decisions in monitoring patients' vital signs and conduct the appropriate follow-up as indicated. Develops programs to improve clinical outcomes and utilization of monitors with exceptional communication skills to deal with physicians, clinicians, and patients. Possesses computer skills so as to be proficient in all aspects of the Monitoring Station and run various reports on a regular basis to support quality initiatives. Performs Community Health Nurse home visits.

Responsibilities:

**EDUCATION**

* Licensure and current registration as a Registered Professional Nurse in the State of New York required

* BSN preferred

* Current BLS certification

**EXPERIENCE**

* One (1) year nursing experience required

* One (1) year experience in critical care, cardiac care, emergency room, and/or home care preferred

**KNOWLEDGE, SKILL AND ABILITY**

* Specialized experience and knowledge of cardiopulmonary and diabetic disease processes

* Pass a physical exam after being offered employment and a health assessment annually thereafter

* Most possess a valid driver's license and have a car available for work use

* Follow MSHC dress code and wear photo identification when serving McAuley Seton patients

* Proficient in keyboarding and computer skills

* Proficiency with Microsoft Office including Word, Excel, and PowerPoint

* Ability to learn Monitoring Station software programs

**WORKING CONDITIONS**

* Office environment

* May be exposed to unsanitary conditions in some home settings

* May be exposed to high crime areas within the community, within the county serviced by the agency

* Endures exposure to weather and temperature extremes
