Job Description:This position functions as a vital member of the healthcare team who contributes to thesuccess of the practice by demonstrating customer/patient focus, effectivecommunication, professionalism, and teamwork. Utilizes the nursing process to assess,plan, implement, and evaluate comprehensive care provided to selected patientpopulations and their families.This position is responsible for timely and effective management of patient callsrequiring medical triage and coordinating care during the transition of care following aninpatient medical stay. Provides guidance, education and facilitates resources for themanagement of the at-risk population.This is a virtual position, serving a region of the health system.POSTING SPECIFICSBenefits Eligible: YesShift Details: Monday - Friday, rotating shifts between 7am - 3:30pm and 8:30am - 5pm, no holidays or weekendsAdditional Details: Must live near a Colorado Intermountain Facility for occasional care site visitsESSENTIAL DUTIES AND RESPONSIBILITIESPerforms clinical triage, including a comprehensive assessment, disposition and education using established triage protocols and guidelinesManages transitions of care to ensure appropriate follow up care and mitigate risks associated with readmission.Contacts patients following emergency department visits to ensure appropriate follow upUses registry data to identify gaps in services and initiates interventionRefers patients to a variety of resources, including behavioral health, transportation services, nutrition, etc.Educates patients and families about health status, health maintenance, and management of acute and chronic conditions.Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations.Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices.Participates in multidisciplinary teams to improve patient care processes and outcomes.Interacts with all staff, colleagues and team members in a professional and collegial mannerPromotes mission, vision, and values of SCL Health, and abides by service behavior standards.EDUCATIONRequired:Graduate of an accredited school of nursingPreferred:Bachelor of Science in NursingCERTIFICATION REGISTRATION, LICENSERequired:Current/valid Registered Nurse license from the state licensing boardBasic Life Support (BLS) or Advanced Care Life Support (ACLS)EXPERIENCERequired:One (1) year related experience in a clinic or acute hospital nursing settingKNOWLEDGE, SKILLS, AND ABILITIESTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
