This job description, defining qualification, primary and secondary functions, responsibilities and duties is intended as a guideline for the employee and manager. The duties listed here are not considered to be all inclusive. The employee in this position performs these and other duties as required although not listed which may be assigned by his supervisor or facility Administrator.
ESSENTIAL FUNCTIONS:
Conducts a Physical Therapy evaluation upon receiving physicians order.
Develops treatment plan based on initial evaluation.
Documents appropriate changes in treatment plan according to patients performance.
Schedules and provides direct patient care utilizing appropriate therapeutic techniques.
Instructs patient and/or patients family members in home program.
Maintains records pertinent to resident treatment:
A. MD order for eval
B. Clarification order
C. Evaluation report
D. Daily tx log
E. Weekly summary
F. 14 day physician notification
G. Discharge summaries
H. Completes Medicare forms appropriately

Assumes responsibility for reading and following all documentation guidelines in the Documentation Manual.
Observes Residents Rights policies at all times.
Maintains 75% - 80% productivity level.
Communicates with nursing and physician regarding changes or modification in treatment plan or patient status.
Demonstrates consistent ability to manage rehab service problems, emergency situations and initiates appropriate action.
Assumes the responsibility for the development of Physical Therapy objectives in conjunction with Director of Rehab.
Complies with federal, state, JCAHO standards and local regulations related to therapy services.
Supervises activities of professional and non-professional rehab personnel.
Participates in patient care rounds, rehabilitation team conferences, conferences with physicians, members of the physical therapy department and/or rehabilitation team, as necessary regarding problems, progress and needs of patient.
Studies current trends and new developments in physical therapy treatment by survey of current literature and attendance of professional meetings and conferences.
Actively involved in providing inservice education to all personnel.
Transports patient to and from the therapy department.
Demonstrates knowledge and understanding of the impact of disability upon an individual's, as well as families' physical, emotional, social, economic, and vocational status, taking into consideration age-related issues, and developmental levels over the life span (i.e., the adolescents' need to develop their own identity and cope with life changes; and the geriatrics' need to maintain their integrity and independence and adjust to the physiological aspects of aging), and meets competency requirements for the population served (adolescent through geriatric), as applicable to the job description.

NON-ESSENTIAL FUNCTIONS:
Assists Director/Supervisor in preparing employee evaluations and recommendations for salary increases, promotions or terminations.
Assists with hiring process; trains and evaluates new personnel.
Counsels and provides input to Director regarding disciplinary process.
Participates in the facility CQI program.
Performs other duties as assigned by the Director of Rehab.
Conducts department tours for interested groups and participates in marketing.
Prerequisites:
Bachelor of Science Degree in Physical Therapy.
Valid state license in Physical Therapy necessary.
Current registration with APTA preferred.
Current CPR Certification required.
Experience preferred, but not necessary.
Knowledge of growth and development across the life span required.
