**Description:**
**POSITION SUMMARY**
Obtains information from patients. Obtains patient signatures as outlined according to procedure. Explains health care policies and procedures to patients. Assigns rooms and prepares records of admissions and outpatient services.
**JOB FUNCTIONS**
+ Obtain personal and financial information and signatures on insurance and consent forms from patients or family members. Witness signature of forms.
+ Assign rooms to patients and explain hospital policies and procedures when necessary. Explain Advance Directives to patients/families. Initiate patient transfers. Escort or make arrangements to have patient escorted to department.
+ Prepares records of admission and outpatient services. Verifies all demographic and insurance information. Prepares charts, labels, and identification bands when applicable.
+ Meet and greet visitors in the outpatient department. Register outpatients as necessary.
**POSITION QUALIFICATIONS**
+ Well developed communication skills, both written and oral. Able to communicate effectively with individuals and groups representing diverse perspectives.
+ Prior employment in a health care environment preferred.
+ Knowledge of medical terminology.
+ Knowledge and ability to use computer applications. Ability to complete data entry functions.
+ Articulates and demonstrates commitment to the philosophy, values, mission and vision of the organization.
**SHIFT**
Variable from 6am-11pm. Some weekends and holidays.
