What you'll be doing

  • In conjunction with the multidisciplinary team, the Program Nurse LPN, the Residential Supervisor and the residents' physicians, oversee the medical treatment and health status of HNNR A.F.C. residents.
  • Prior to residential admission, contact hospital R.N. to obtain current information regarding health status, medications, treatments and care needs; May provide Eyes-on Assessments.
  • Conduct initial and as needed Health Care Assessments of residents.
  • Writes care plans on residents whose health care status is complex. Specify treatments that are to be given to and procedure and protocols that are to be followed with individual residents. Monitor follow-through and report problems to the Residential Supervisor.
  • Monitor and evaluate residents' medications. Work with the program case manager to obtain prescriptions for medications, treatments, special diets and needed equipment. Monitor medication administration record sheets and prescription labels as obtained from the pharmacy.
  • Document medical treatment and health status in the residents' records for continuity of care and treatment reference.
  • Coordinate and implement infection control procedures and other health related protocols and procedures that protect the consumers and staff, and their associates.
  • Provide leadership, support, and direction to staff to facilitate teamwork, problem solving, and program development. Model and promote positive morale.

Anticipated Work Schedule

Monday-Friday 8:30am-5:00pm with some flexibility

Job Requirements

  • Bachelor's degree in nursing or as required for licensure as a registered nurse; licensed to practice in State of Michigan.
  • One-year previous experience in a residential setting as an RN preferred.
  • Experience providing medical services in a rehabilitation setting.
