PACE (Program of All-Inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.

Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.

POSITION SUMMARY:

The Rehabilitation Aide provides functional maintenance therapy services to On Lok participants under the clinical supervision of Physical and Occupational Therapists. This role may report directly to the Program Manager, Center Support Supervisor or a Clinical Specialist, depending on the Center configuration.

DUTIES / RESPONSIBILITIES:

* Provide Functional Maintenance Therapy (FMP) according to the individual treatment plan in therapeutic exercises, therapeutic activities, therapeutic groups, and/or other activities as directed by the Physical Therapist and/or Occupational Therapist. Report observed changes in the participant's condition to supervising therapists.

* Record daily treatment service in the rehabilitation Monthly Service Record (FMP Flow Sheet) accurately and in a timely manner.

* Prepare the rehab Monthly Service Record (FMP Flow Sheet) accurately according to PT/OT treatment plans.

* Maintain the tidiness and cleanliness of therapy equipment and devices according to agency cleaning protocols.

* Monitor and check the rehab gym for adequate equipment and supplies. Assure that equipment is in operating condition and communicate to therapists any damage or other indications of malfunction in need of maintenance.

* Demonstrate safe practices by utilizing proper body mechanics, adhering to universal precautions, and maintaining environmental safety.

* Promote the benefits of therapy and motivates participants in therapeutic exercise programs.

* Orient and train new employees to FMP and equipment as requested by the Physical/Occupational Therapists and/or Specialists.

* Provide direct personal care services to participants as needed and assigned by the supervisor.

* Contribute to center operations by demonstrating leadership, collaborating with interdisciplinary team members, participating in team discussions, and supporting team treatment plans.

* Other related duties as assigned by Program Manager or Physical/Occupational Therapists.

QUALIFICATIONS (knowledge, skills, abilities):

* Demonstrated interest, ability, and at least one year of experience in working with the elderly or frail population.

* Demonstrated interest, enthusiasm, skills, and ability to work in the specialty area of rehabilitation (therapy services).

* Ability to work independently and as part of the interdisciplinary team.

* Ability to use basic verbal and written English skills to communicate with participants, colleagues, and supervisors. Bilingual ability preferred.

* Computer literacy for clerical, administrative, and/or clinical data tasks.

* Physical activities require the ability to stoop, reach, stand, walk, push, pull, lift, grasp, and/or other movements associated with job duties.

* Physical Requirement: Ability to handle direct participant care related to job duties within the parameters of organization's Safe Participant Handling policy.

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
