  • Responsible for gathering data, documenting and presenting each member's goals and objectives in accordance with ISP regulations; coordinating services with case managers and parent/guardians of program member's goals and objectives, issues, concerns and abilities; and ensuring the provision of high quality services. The Rehabilitation Specialist is also responsible for overseeing the screening and prioritizing of referrals; documenting member progress; implementing behavior management strategies; investigating incidents and all other duties as assigned.
Qualifications:
  • High School diploma with 5 years related experience OR Associate's degree with three years related experience OR Bachelor's degree with one year related experience.
  • Knowledge of and respect for diverse populations.
  • Previous experience working in human services preferred.
Requirements:
  • 2 years of previous case management or care management experience required.
  • Ability to use computers to include Microsoft Word, data entry, email and internet essential.
  • Experience in Credible preferred.
  • Must be organized, detail-oriented, have solid time management skills and be able to work in a fast-paced multi-tasking environment.
  • Demonstrated ability to deliver high level of customer service/satisfaction with both internal and external customers.
  • Excellent communication, interpersonal and listening skills.
  • Must have a valid driver's license, good driving record, use of vehicle and willingness to transport participants in personal and/or agency vehicle.
