The average salary for the position is $40,000.We are now hiring for a full-time Case Manager to join our South Chicago Reimagine Youth team!ESSENTIAL JOB FUNCTIONS:
  • Using intermediate level skills, conducts comprehensive client assessment at point of intake to determine client desires, strengths and needs.
  • Develops a written action plan with the client to meet client desires and needs, specifying goals to be achieved and services to be pursued.
  • Provides advocacy, case management, and supportive mentoring and counseling to clients and their families to encourage success. Services provided may include social, educational, and vocational activities within MFS or in the community (schools), either in a group setting or individually.
  • Uses expanded knowledge of resources and helps to link and engage clients to resources of client interest.Works collaboratively with community resources to provide effective wraparound services to ensure client success.
  • Handles emergency and crisis situations within target population.
  • Works within contract, grant, and program requirements including meeting timelines and reporting requirements.
  • Reassess and monitors client situation and adequacy of service plan to determine level of continuation of services or discharge to community resources.
  • Collaborates and participates with other program staff in the planning and implementation of service plans.
  • With supervisory approval, authorizes limited amounts of financial assistance for client need and for overall improvement of Social Determinants of Health.
  • Identifies systems issues and reports to supervisor as appropriate. May work with community groups and may carry a special community assignment.
  • Takes increasing responsibility for own professional development, utilizing supervision and other developmental opportunities within and outside the agency to enhance skills and understanding.
  • Maintains competencies in working with diverse populations in order to successfully engage clients in assessments, service/care planning and referral and linkage.
  • Completes documentation/data entry on services rendered, attempted contacts, staffing, supervision, etc. in EHR and health population platforms in a timely manner.
  • Works effectively with diverse staff and service populations.
  • Other duties as assigned.
QUALIFICATIONS:
  • Bachelor's Degree in Social Work or related field from an accredited college or university required.
  • One year of case management experience in a social service or healthcare setting required.
  • Experience with youth/adolescents in the juvenile justice system, preferred.
  • Bilingual in Spanish preferred.
  • Crisis stabilization experience preferred.
  • Demonstrated ability to render adequate service to clients and to organize and manage job efficiently.
  • Must have access to a vehicle, valid driver's license and proof of insurance to perform essential job functions, required.
  • Ability to maintain assigned community and home-based services.
  • Ability to work effectively with diverse individuals and groups.
  • Ability to work evening and weekend hours as needed.
  • Desire to utilize creative ideas and solutions to encourage and motivate clients for success in their community.
PHYSICAL DEMANDS:
  • While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch.
  • The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.
  • The employee may occasionally lift and/or move up to 50 pounds.
