Compass Healthcare Consulting & Placement is conducting a search for a Full Time Remote - NYS LMSW Social Work Care Manager for our client a Managed Long Term Care Plan. Qualified candidates will have a NYS LMSW and prior LMSW Care Management experience from a Managed Long Term Care Plan (MLTC). Bilingual Chinese, Mandarin/Cantonese, Spanish, Russian, Bengali Skills, a plus. Position is Fully Remote, Work from Home.
This is a full time Direct Hire position, Monday to Friday.
About the position
The LMSW Social Work Care Manager provides Telephonic Care Management services to Managed Long Term Care Members. LMSW SW Care Manager provides emotional support to Members and families as well as referral optional and long term healthcare planning options. LMSW BH Care Managers are key to members maintaining a safe and effective home care plan.
Qualifications:
1) NYS Licensed Masters Social Worker LMSW or LCSW, Licensed Clinical Social Worker
2) Prior Managed Care Plan, (MLTC) Experience required
3) Case Management experience Required
Competitive Salary $80,000 - 85,000 and Excellent Benefits, PTO, Medical/Dental, Pension Plan, Salary Based on years of MLTC experience and bilingual language ability.
