Removing Barriers to Healthcare AccessGreenway AdminThe Population Health Specialist (PHS) is responsible for the oversight and coordination of population health topics and tracking of abnormal cancer screening results, to drive improvement in clinical quality outcomes and ensure patient safety. The PHS duties vary based on area of focus but span topics like outreach for preventative care and chronic disease management, ordering cancer screening tests, scheduling appointments, reviewing abnormal results, coordinating with care teams, data clean up, and more. The PHS supports organization-wide improvement processes by assisting standing and ad hoc Quality Improvement (QI) Teams and initiatives with data collection, correction, retrieval, and reporting and works with ODCHC providers and staff to support high quality care delivery.Compensation Range:$30.00-$34.84All new hires will begin at the base wage of this position.Hmong and Spanish speakers who successfully pass a language exam will receive an additional .75 hourly differential to their wage.ESSENTIAL DUTIES AND RESPONSIBILITIES: These essential responsibilities are performed within the context of collaboration, coordination and team-building within the QI Department and among the staff and management requesting and/or using QI products:
  • Audits data for compliance; assesses common data errors and gaps and works with EMR Team to identify areas needing improvement.
  • Per protocol, reviews and updates patient health maintenance information.
  • Per protocol, orders preventative screenings per health maintenance status.
  • Per protocol, reviews and updates provider panels, including patient assignment and reassignment.
  • Enters and organizes information in Excel and other data collection and summary tools.
  • Assists in the preparation of quality improvement reports and technical evaluations.
  • Participates on committees, teams, and with colleagues as needed.
  • Participates in Open Door Quality Improvement activities, meetings, and initiatives.
  • Performs other duties as identified and assigned supervisor.
Outreach Focus
  • Follows standard operating procedures to run reports used to create outreach campaigns for vari-ous outreach topics.
  • Manages daily outreach campaigns including: preparing, sending, monitoring responses, and, tracking outreach campaigns.
  • Manages patient outreach responses, places orders for preventative cancer screenings such as mammogram and FIT tests, schedules appointments, answers patient questions, connects pa-tients with their care team when needed and updates empanelment information as needed.
  • Follows Open Door's population health outreach schedule, updating schedule with any changes, coordinates posting to the WebPortal with the Communications team and communicating changes to health centers.
  • Collaborates with the Quality and health center leadership teams on custom outreach campaigns or requests.
  • Supports identified health center staff tasked with support outreach at the health center level, serving as the subject matter expert for population health outreach through Artera.
  • Coordinates support for training and education for health center staff in Artera with the EHR team as needed.
  • Coordinates with the Clinical Quality Analyst (CQA) to pull Artera undeliverable reports used to conduct outreach in alternate modes such as, MyChart, letter, or phone.
  • Supports population focused outreach in an effort to close disparities in care in specific populations where notable gaps in care are identified.
Quality Assurance Focus
  • Follows protocols to ensure appropriate follow up plans are developed in alignment with the cur-rent clinical guidelines and governing body, and patient records are updated appropriately.
  • Supports care teams and providers in updating patient records correctly when cancer screening results are received, both abnormal and normal.
  • Collaborates with Clinical Services and EHR teams in training topics related too cancer results management.
  • Collaborates with clinic staff in a timely manner to assist and coordinate site level patient outreach efforts related to follow up after abnormal results.
QUALIFICATIONS AND EXPECTATIONS: The successful candidate will possess experience and skills spanning a variety areas:
  • Effective verbal and written communication skills; ability to interact and work as part of the QI Team and larger ODCHC organization.
  • Strong interpersonal skills and the ability to establish constructive professional working relation-ships among a diverse workforce.
  • Ability to write basic descriptions, explanations, instructions, and other correspondence.
  • Ability to prepare routine reports, effectively present information, and answer questions.
  • Ability to read, interpret and implement moderately complex documents, policies, and program specifications.
  • Strong computer skills with specific aptitude in Microsoft Office Suite, specifically Excel, and/or Electronic Medical Records (EMR).
  • Computer and computer application skills sufficient to accurately and efficiently use practice management system, electronic health records system, spreadsheets, email and word processing software.
  • Ability to manage and complete multiple projects simultaneously.
  • Ability to work independently and as a member of multiple teams or groups.
  • Knowledge of Continuous Quality Improvement principles and tools preferred.
  • Adheres to ODCHC policies and protocols.
EDUCATION AND EXPEREINCE:
  • High school diploma or GED; Associate or Bachelor Degree preferred.
  • Prior experience of at least two years in healthcare, as a medical assistant or other back office support; experience with ODCHC systems and protocols preferred.
CERTIFICATES, LICENSES, AND SKILLS:
  • Credentialed (Certification or Acknowledgement) as a Medical Assistant
SUPERVISORY RESPONSIBILITIES: The Population Health Specialist is not a supervisory position.SUPERVISION AND SUPPORT: The Population Health Specialist reports directly to the Quality Improvement Managers and works collaboratively with Quality improvement and Population Coordinator and other Population Health Specialists. Extensive collaboration with other clinic staff and managers is expected.PHYSICAL REQUIREMENTS: This is largely an office-based position. The physical requirements de-scribed are representative of those needed to successfully perform the essential duties of the position.
