JOB DESCRIPTION
Remote Patient Monitoring (RPM) Coordinators play a crucial role in managing remote patient care, bridging the gap between technology and clinical practice. They help ensure that patients receive timely interventions and that healthcare teams have the data they need to make informed decisions. RPM Coordinators serve patients by proactively monitoring, educating, and coaching patients on how to manage their acute or chronic medical conditions, with a goal to maintain health, wellness, and to avoid emergency room utilization and hospitalization.
Responsibilities
- Collaboratively planning sessions with patients to define patient-centric goals and the development of associated action plans to improve their health and well-being
- Monitoring real-time patient health data to identify irregularities and concerning trends that need to be escalated to clinical staff
- Liaising between patients, caregivers, and healthcare providers to ensure adherence to treatment plans and remote monitoring protocols
- Scheduling and coordinating remote and in-person appointments based on the patient's health status and monitoring results
- Tracking, reporting, and maintaining detailed documentation of patient interactions, device data, and care plan updates while ensuring compliance with healthcare regulations regarding patient data and confidentiality
- Coordinating the enrollment of patients into the RPM program, educating them on the use of monitoring devices, assisting with device setup, and troubleshooting/providing technical support
- Preparing reports and data summaries to analyze trends in the data collected and inform healthcare teams of any preventative actions that should be taken
Qualifications
- At least 1 year of experience as a Medical Assistant, HHA, PCA, or Nursing student
- Desire and ability to work in a remote work environment within a fast-paced culture
- Self-motivation and the ability to work independently with minimal direction
- Experience performing virtual visits with patients and telephonic care management
- A high degree of professionalism, compassion, and respect for patient privacy
- Strong verbal and written communication skills
- Must be able to work a 9am-5pm schedule
- Must have experience with Medicare patients
- Bilingual English/Spanish required
