Job DetailsReporting to the Supervisor of Care Management this position will provide MLTC case management to our members including medical, social, psychological, physical, and spiritual needs.RN Case Manager will develop, implement and monitor the care plan, in conjunction with the PCP, caregivers and other team members, to help meet the member's needsThis is a RMOTE RN Case Manager role and REQUIRES the RN to live in the NYC Metro area to be considered for employment. RN Must have active New York State RN license in good standing. RN will be required to TRAIN in the NYC office for the first 3 months of employment and will then transition to telecommute after successfully completing 3 month probation period.
