- The essential functions of the job for the Care Manager IV- RA, requires, and may be amended from time to time at the sole discretion of Viva Senior Living to include, but are not limited to the following:
- Comply with standards of business conduct in accordance with federal, state and local health and regulatory standards and guidelines, as applicable.
- Must possess, at a minimum, a high school diploma or a GED
- Must be able to work well with peers in a congenial work environment, and above all, maintain the utmost compassion and dignity to the residents and adhere to the Resident Bill of Rights.
- In exigent circumstances, the employee may be subject to call back during emergency conditions (e.g., severe weather,
evacuation, temporary staffing shortages, etc.).
- Must adhere to Viva Senior Living's attendance policies and follow the terms and conditions of employment as set forth in the Employee Handbook.
- Must be able to read, write, speak and understand English.
- Must maintain the highest standards in caring and servicing the needs of the residents and residents' family members and loves ones.
- Must adhere to all facility policies and procedures.
RESPONSIBILITIES/ACCOUNTABILITIES
- Supervise and care for residents on daily basis according to your work list and assignment
- Provide assistance with activities of daily living according to resident's individualized Care Plan, regarding bathing, shower, shampoo if necessary, make bed, linens, escorts, assistance with dressing, oral hygiene, shaving, and hair and fingernail care if needed
- Observe and record assistance with prescribed medications, as per assignment
- Report refusals and/or reactions to care or medications to supervisor on duty
- Direct residents and family members to appropriate staff for other needs or information
- Assist with ordering room service for residents, when needed
- Report and record observations of change in resident condition to director/ supervisor and transfer relevant information to the next shift through the 24-hour shift report
- Be patient and understanding when communicating with staff, residents, and visitors. Speak slowly and clearly
- Provide assistance in dining room, as needed
- Answer emergency calls PROMPTLY and courteously, and complete required paperwork afterward
- Retrieve soiled linen and personal clothing from resident living areas. Sort and pre-soak linens, as required, before processing through washing machines, per resident's Plan of Care
- Responsible for general upkeep and cleanliness of a resident's apartment/ room. Empty waste baskets, wipe up spills, wash dishes, put away clothes, check refrigerator for spoiled food
- Report any unsecured throw rugs, extension cords, and any other fire/ fall hazards to maintain the No Fall Zone
- Observe condition of room/apartment and report anything in need of repair to maintenance/ supervisor
- Document clearly and objectively on residents
- Participate in in-service educational programs, as required by the community and state.
- Record all care given and refusals
- Maintain required safety/ building checks
- Obtain resident weights, as needed
- Maintain universal precautions; bloodborne pathogens and biohazard precautions
- Promote positive public relations for the Community
- Provide assistance with toileting, personal care, changing incontinence products
- Assist with use of durable medical equipment (DME) to include walkers, wheelchairs, and canes
- Use good body mechanics to provide occasional assistance with transfers
- Assist residents to perform passive range of motion (ROM) exercises, as per Plan of Care
- Perform other duties, as required
RequirementsAnnual Health Requirements:
- Must provide annual verification of a negative TB skin test.
- Proof of full COVID-19 vaccination required with exceptions for medical and religious accommodations.
Physical and Sensory Requirements: (With or Without the Aid of Mechanical Devices)
- Must be able to move intermittently throughout the workday.
- Must be able to cope with the mental and emotional stress of the position.
- Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
- Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
- Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, agitated residents
- Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
- May be necessary to assist in the evacuation of residents during emergency situations.
