Job Overview:
As a Resident Care Advocate, you will play a vital role in ensuring that residents receive personalized care that meets their physical, emotional, and social needs. You will serve as a liaison between residents, families, and staff, advocating for the rights and well-being of each resident while promoting a culture of respect and dignity.
Key Responsibilities:
- Resident Engagement: Build meaningful relationships with residents to understand their needs, preferences, and concerns, promoting their voice in their care.
- Care Coordination: Collaborate with interdisciplinary teams to develop and implement individualized care plans, ensuring that they align with residents goals and preferences.
- Advocacy: Act as an advocate for residents and their families, addressing any issues or concerns related to care, services, or community living.
- Communication: Facilitate regular communication between residents, families, and care staff to keep everyone informed about resident health and wellbeing.
- Support and Education: Provide information and resources to residents and families about available services, programs, and community resources.
- Quality Assurance: Participate in quality improvement initiatives, helping to ensure that care standards are met and residents are satisfied with their care experience.
- Crisis Management: Respond to emergencies or changes in resident health status, coordinating appropriate interventions and resources.
Qualifications:
- Education: Bachelors degree in Nursing, Social Work, Gerontology, or a related field preferred; relevant experience may substitute for education.
- Experience: Previous experience in a healthcare or senior living environment is preferred, with a focus on resident advocacy or care coordination.
- Skills: Strong interpersonal and communication skills, excellent problem-solving abilities, and a genuine passion for working with seniors.
- Knowledge: Understanding of aging processes, dementia care, and applicable regulations in assisted living.
- Certifications: CPR and First Aid certification required; additional certifications in geriatric care or advocacy are a plus.
