Dallam Hartley Counties Hospital District invites you to become an integral part of our team! We are currently looking for a Medical Records Clerk/Transportation Aide to provide essential clerical support to the Nursing Home Administrative team, as well as, transportation services for our nursing home and assisted living residents. Essential Responsibilities:
- Enter incident reports and give a copy to the Director of Nursing.
- Track ER and Hospital visits.
- Track status changes including discharges, room transfers, Hospice, and hospital admissions.
- Record doctor's visits, inform the Director of Nursing of any pertinent changes in patient's conditions.
- Transportation to internal facilities, and external facilities in Amarillo
- Assisting with aide like duties, for example: toileting, transferring, etc.
- Appropriate record keeping and filing of charts.
- Create census sheets.
- Track forms needed on facility wings and deliver to the appropriate wing.
- Retain packets for admissions and have them readily available for New Admissions.
- Schedule all appointments and inform family members of resident's appointments.
- Monthly reporting.
- Record monthly Physician orders.
- Have all necessary documentation in a binder for residents in each facility wing.
