Job Summary: The Resident Service Coordinator will provide service coordination, assessment, case management, advocacy and linkages to seniors living in the Mission Villas Community. The Service Coordinator will establish linkages with agencies and service providers in the community who serve seniors. The Services Coordinator will work and consult with property management staff and provide education to property management or other staff regarding the needs and appropriate service delivery to this population.

Essential Duties:

1. Provide intake, assessment and referral services for seniors

2. Provide ongoing case management for those individuals who need this service to link with and utilize existing community resources;

3. Establish and maintain linkages with service providers in the community;

4. Monitor the ongoing services that are delivered to residents by community providers to assure that they are appropriate and that the resident is progressing;

5. Develop and implement individual service plans for each resident who receives ongoing case management;

6. Maintain accurate, timely and confidential documentation on all clients served;

7. Assist residents to develop informal support networks with family, friends and other residents;

8. Educate other staff and property management team about the needs and services available for seniors;

9. Program development

a. The development and implementation of programs including but not limited to:

i. Health and Wellness programming

ii. Senior programming

iii. Social and recreational programming

iv. Special events

10. Coordinate outreach as needed for the senior nutrition program, senior exercise program, and other classes provided at the senior center;

11. Develop and maintain an onsite resource guide of service providers available to the residents;

12. Serve as an advocate for residents to ensure that they are receiving all services for which they are eligible;

13. Provide monthly input e.g., statistics, number of clients served, center activities/classes

Reports to: Program Manager.

Minimum Qualifications

  • .A minimum of three to five years of experience in social service delivery with an interest in senior citizens.
  • Bilingual required (Spanish/English) with excellent oral and written communication skills.
  • Working knowledge of Federal, State, and local laws and requirements related to elderly and disabled programs, as well as non-governmental networks related to geriatric services, e.g. homemakers, Meals-on Wheels, disability counseling, and local Agencies on Aging.
  • Understanding of the physical, mental, and social changes associated with elderly and disabled populations, including cognitive impairments, medication/substance abuse, fraud/violent crime awareness and self protection, and dealing with death and loss.
  • Demonstrated working knowledge of supportive services for senior citizens and adults with disabilities available in the local community.
  • Demonstrated ability to advocate, organize, problem-solve, and obtain results for seniors;
  • Computer literacy Microsoft Office and case-management software.
  • Licenses & Certifications:

    • B.A. in social work or related field required
