POSITION TITLE
Resident Service Coordinator

POSITION SUMMARY
The Resident Service Coordinator will serve as a part of the supportive services team and will focus on building a system to support collaborative relationships with the residents to impacttheir health, well-being, and financial stability goals. The Resident Service Coordinator will play a key role in the intake process of potential residents as well as resource coordination services. In addition, the Resident Service Coordinator will collect data and analyze data to drive program decisions. The Resident Service Coordinator will provide case management and referral coordination services, and otherwise support peaceful community living.
MINIMUM QUALIFICATIONS
* Bachelor's degree in Social Work or related field, a minimum of 3-5 years of recent related experience.
* Associates degree with at least five years of recent related experience.
* A valid Driver's License and vehicle insurance coverage required.
* Occasional late or weekend hours may be required, exceptional oral and written communication skills, excellent engagement skills and the ability to relate and work effectively with a diverse group of people from all social and economic backgrounds.
* Candidates must have some computer skills.
RESPONSIBILITIES
* Delivering care coordination services as needed (resident assessment, advocacy, planning, facilitation, monitoring, and evaluation of options and resources to meet a resident's specific need.)
* Care coordination and referral services in the areas of physical and emotional needs, health, substance abuse treatment, employment and other resources as needed.
* Coordinating and/or leading workshops or other classes on life skills, employment readiness and job searches, coordinating or applying for benefits and entitlement programs, financial literacy, health and wellness
* Planning and coordinating monthly events
* Participating in monthly meetings with the Property Manager and other key staff to discuss trends and processes to improve the overall delivery of services
* Maintaining resident service documentation, logs and notes while ensuring resident confidentiality.
* Conduct satisfaction surveys with the residents, compile and present survey data for recommendations and process improvements
* Prepare and distribute a monthly calendar of events for the residents
* Prepare and submit monthly service reports to direct supervisor
* Participate in supervision, CCHC quality initiatives and other staff meetings as required
* Performs other related duties as assigned by direct supervisor
