Responsibilities:
- Conduct intake appointments upon resident move-in
- Meet with residents on a regular basis.
- Create and case manage resident service records.
- Enter data and outcomes into an electronic reporting portal.
- Build and maintain relationships with community service providers, property management and other team members.
- Coordinate supports and promotes positive outcomes.
- Assist residents in accessing community resources.
- Facilitate resident meetings.
- Encourage residents to be proactive toward addressing social, behavioral, and physical needs, maintaining self-sufficiency and housing stability.
- Educate residents on service availability, application procedures, and residents rights and responsibilities,
- Facilitate-e volunteer support, educational and recreational opportunities with community providers.
- Other duties as assigned.
Qualifications:
- A B.S. degree in a social services field is preferred, Applicants with of a minimum of five years' experience in human/social services or a combination of experience and education will also be considered.
- An understanding of social service systems of care and accessing community resources is essential.
- Reliable transportation and willingness to travel to both required sites.
- Strong verbal and written communication skills needed.
- Experience with file documentation and case management required.
