Job Description:

Summary of Duties

Responsible for providing residents with nursing care and services in accordance with the resident's assessment and plan of care and as directed.

Essential Job Functions

1. Provides services to residents including activities of daily living.

2. Participate in the orientation of new residents to the facility including obtaining resident information.

3. Assists with and/or arrange services, appointments, recreational activities, etc. for residents as directed.

4. Assists with the admission and discharge of residents.

5. Documents services provided as required.

6. Reports all changes in the resident's condition (i.e., areas of redness, skin tears, bruising, mental status change, eating habits, etc.) to the Nurse Supervisor/Charge Nurse as soon as practical.

7. Performs restorative and rehabilitation procedures as instructed.

8. Checks each resident regularly to ensure that his/her needs are being met.

9. Assists in transporting residents to/from dining room, therapy, activity and social programs, etc., as necessary.

10. Encourages residents to participate in socialization activities.

11. Assists with the investigation of resident complaints and grievances as directed.

12. Assists with obtaining supplies and equipment for residents as directed.

13. Performs resident care including lifting, turning, moving, positioning, and transferring residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., as necessary, utilizing facility technique.

14. Keeps resident rooms and resident care areas clean and neat.

15. Attends resident care plan meetings as directed.

16. Participates in Quality Assurance Performance Improvement (QAPI) activities as requested.

17. Performs other duties as directed and assume additional responsibilities as assigned.

Education

Must be a Certified Nursing Assistant or obtain certification within 4 months of hire. High school diploma or equivalent preferred.

Experience

Certified Nursing Assistant experience preferred.

Two (2) years experience in long term care preferred.

Specific Requirements

1. Must be a Certified Nursing Assistant or obtain certification within 4 months of hire.

2. Must be able to demonstrate basic computer skills within thirty days of employment.

3. Must be able to read, write and speak the English language.

4. Must possess the ability to make independent decisions when circumstances warrant such action.

5. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

6. Must work harmoniously with other personnel.

7. Must be able to relate information concerning a resident's condition.

8. Must possess flexibility.

9. Must be able to follow oral and written instructions.

10. Must demonstrate sensitivity to the special needs of residents.
