Job Title: Resident Services Coordinator Job Summary The Resident Services Coordinator position will be responsible for linking the elderly residents, especially those who are frail or at-risk to the supportive services they need to continue living independently.Service Coordination means the activity of linking a resident to needed supportive services or medical services which may be provided by agencies in the general community.The Resident Services Coordinator assesses service needs determines eligibility for public services. Essential Job Functions
- Provides general case management (including intake) and referral services to all residents needing such assistance.
- Provides formal case management (i.e., social, and emotional needs, development of an individually tailored case plan for services and periodic reassessment of the resident's situation and needs) for a resident when such service is not available through the general community.
- Establishes linkages with all agencies and service providers in the community; shops around to determine/develop the best deals in service pricing to assure individualized, flexible and creative services for the involved resident(s).
- Sets up a directory of service providers for use by both project staff and residents.
- Refers and links the residents of the project to service providers in the general community; these are, case management, personal assistance, homemakers, meals-on-wheels, transportation, counseling occasional visiting nurse, preventative health screening/wellness and legal advocacy.
- Educates residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate.
- Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual.
- Sets up volunteer support programs with service organizations in the community as needed.
- Helps the residents build informal support networks with other residents, family and friends.
- Provides training to project residents in the obligations of tenancy or coordinate such training as needed.
- Respond to management requests regarding resident quality of life projects and special assignments.
- Prepares monthly newsletters and calendars.
- Arranges for social events, meetings and parties.
- Participates in department meetings and activities as necessary.
Job Requirements Education:
- Bachelor of Social Work or Degree in Gerontology, Psychology or Counseling is preferable; a college degree is desirable.
Experience:
- One to two (1-2) years' experience in social service delivery with senior citizens and non-elderly disabled.Supervisory and management experience is desirable
- Training in the aging process, elder services, disability services, eligibility for and procedures of federal and applicable state entitlement programs, legal liability issues relating to providing service coordination, drug and alcohol use and abuse by the elderly, and metal health issues is desirable
- Demonstrated ability to advocate, organize, problem-solve and provide results for the elderly and disabled.
Licenses/Certifications:
Abilities Required
- Knowledge of word processing and printing capabilities
- Excellent communication, interpersonal and communication skills with residents and potential clients
- The ability to work well with residents
- Ability to communicate in both English and Spanish
Functional Demands Environment Work Conditions:
- Normal working condition with adequate lighting and ventilation
Infectious Material Exposure:
- No routine exposure to blood/body fluids/tissues but may occasionally require performing unplanned activities which may involve exposure to blood/body fluids/tissues.
