Job Description:
Nature of Work: The Residential Care Coordinator is responsible for enhancing the quality of life for individuals served and promoting a positive work environment for Direct Support Professionals. The Residential Care Coordinator will accomplish this work through collaborative efforts with other direct care staff and the Residential Manager. With the support of a residential team the Residential Care Coordinator is responsible for maintaining successful programs and healthy milieu for staff and individuals served. The Residential Care Coordinator's work is driven by the agency's mission to improve the quality of life by supporting independence.Essential Functions Consumer Support
  • Be adequately prepared for and attend all PCP and team meetings.
  • Provide input to develop and implement all written Individual Service Plans.
  • Ensure that consumers receive medical, psychiatric and other services as necessary and follow informed consent procedures.
  • Coordinate consumer appointments and transportation needs.
  • Attend psychiatric, urgent and critical care appointments, as needed.
  • Create weekly activity schedules that follow individual PCP plans and adhere to HCBS requirements.
  • Identify different communication abilities of individuals and support/assist them in participating in their PCP and other team meetings.
  • Maintain confidential individuals served information files.
  • The Residential Care Coordinator will be required to provide the services of a Direct Support Professional as needed.
  • Attend all trainings as needed to obtain and maintain certifications.
Safety
  • Perform routine checks of programs and maintain oversight in collaboration with the Residential Manager as assigned.
  • Ensure the completion of vehicle maintenance, annual vehicle registrations and monthly vehicle drills.
  • Conduct fire drills according to required timeframes and frequency.
  • Create a safety awareness culture by role-modeling safe behavior and discussing safety-related issues with staff.
  • Inform staff when unsafe procedures and behaviors occur.
  • Correct and/or report to appropriate supervisor any safety issues.
Documentation
  • Respond to daily documentation audits with a plan of correction for DSP's as assigned.
  • Submit monthly paperwork in adherence to deadlines.
  • Maintain and support consumer financial plans and records.
  • Respond to QA audits within given timeframe.
  • Complete weekly service unit report for admin hours.
Leadership
  • Maintain open and professional communication with all employees, colleagues, supervisors, consumers and outside parties.
  • Complete bi-monthly competency checklists with DSPs within assigned program.
  • Complete a comprehensive DSP orientation for all staff within assigned program.
  • Participate in Residential meetings as scheduled.
  • Provide timely performance feedback to DSPs coaching/training notes.
  • Provide feedback to the Residential Manager to inform accurate 90-day and yearly DSP performance evaluations.
  • Ensure trainings are entered on WhentoWork and promote accountability in training attendance with all program staff with delivery of coaching/training notes as needed.
  • Provide 24/7 on-call support to residential programs as assigned.
  • Follow all policies and protocols to maintain a safe, healthy, and therapeutic environment.
  • In collaboration with the Residential Manager, maintain weekly work schedules which minimize overtime, meets parameters of authorized program hours.

RequirementsPhysical Requirements:While performing the duties of this job, the employee is occasionally required to stand; sit; walk; use hands and fingers; handle; reach with arms and hands; stoop; kneel; crouch; crawl; talk or hear; taste or smell. Must be able to lift and/or move at least 75 pounds to assist in life skills, such as bathing, physical rescue, and wheelchair assistance. Specific vision abilities may include close vision; color vision and the ability to adjust focus. Exposure to outside weather conditions while working with consumers. Noise level in the work environment is usually moderate to occasionally loud.Qualifications:
  • Minimum of two years' experience in the field of human services.
  • High School Degree or GED equivalent.
  • Possess the ability to lead staff in a positive and effective manner.
  • Possess professional values and ethics; model confidentiality practices as stated in federal, state and GMS policies.
  • Possess basic computer skills and a working knowledge of Microsoft Word programs. Experience with Microsoft office products preferred.
  • Possess a valid Maine state driver's license.
  • Prior experience as a DSP and CRMA preferred.
  • Possess good verbal and written communication skills.
  • Ability to obtain and maintain all required certifications.
  • Ability to pass required background checks, to include: Adult/Child Protective Services; criminal history; OIG Exclusion list; National Sex Offender Registry; Bureau of Motor Vehicles record; and Maine CNA Registry.
Salary Description $22.00-$25.00 Hourly
