DescriptionESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Adheres to the policies and procedures of Eagle Village Residential Services and the State of Michigan.Aids in the development and implementation of comprehensive treatment plans for youth in care.Assists treatment team members, with tasks as necessary and assigned, to provide for meeting the needs of youth, contracts and license requirements.Maintains weekly therapeutic contact (phone, email, and/or face to face) with each youth, their families, and their worker regarding progress updates and logistics communication.Responds daily to workers, GALS, families, etc. (phone, email, and/or face to face). Complete case notes after phone calls and face to face interactions.Completes case notes when meeting with youth.Attendance and active participation in monthly Family Legacy's, Team Meetings, Quest, and Family Team Meetings-FTM's, Team Decision Meetings - TDM's, Residential Placement Unit - RPU meetings, Wraparound meetings, and court hearings as needed.Arranges transportation for home visits, family visits, Legacy's, and/or court as needed.Aids in the administration and submission of the Family Needs and Strengths Assessment, Child Needs and Strengths Assessment, data collection tools and life skills.Possess knowledge in trauma especially as it related to child welfare and be knowledgeable in Family Systems Theory, PBIS and other evidence based services utilized by Eagle Village.Collaborates with the Family Therapist, Residential House Supervisor and Experiential Therapist to enhance treatment plan implementation.Completes all required paperwork from intake to discharge within established required time frames, including the preparation and submission of Fast Fact Sheets, Preliminary Service Plans, Initial Treatment Plans, Updated Treatment Plans, Release Plans, CAURT reports, and Court reports.Completes Genograms, Ecomaps, and Developmental Histories with youth, workers, and families.Tracks and obtains signatures from youth, workers, and parents on Initial Treatment Plans, Updated Treatment Plans, and Release Plans.Completes medication review notes, attend all medication reviews, and complete case notes.Uploads documents to youth's files and complete monthly Case File Reviews.Represents Eagle Village as needed at court hearings involving the clients, which may be in the form of in-person appearance, video conferencing, or phone.Participates in a wilderness experiential components as needed.Attends hold debrief meetings as requested.Duties commonly required providing service during some evenings and weekends; crisis intervention or short-notice response may be necessary.Works in coordination with After Care for follow up data entry, visits, phone calls and after care coordination services.Working knowledge and adherence to the Eagle Village policy and procedure manual and the Employee Handbook such as participation in required training and staff meetings, on-call rotation, etc.Adheres to and promote the agency's mission and values in the execution of duties while recognizing and appropriately responding to the cultural differences among those served, as well as fellow staff members.Maintains timely completion of all required orientations and in-service training.Performs any other reasonably related duties assigned by management and therapists.May be asked to fill in as direct care staff as needed and be part of ratio.Transports youth as needed.SUPERVISORY RESPONSIBILITIES:This job has no supervisory responsibilities.RequirementsQualifications:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Knowledge, experience, and skills in working with children and families who have experienced trauma. Be of good moral character. Must be able to type fluently with knowledge of general Microsoft programs.Language Skills:Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from group of managers, clients, customers, and the general public.Mathematical Skills:Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.Reasoning Ability:Ability to solve practical problems and deal with a variety of concrete variables in situations where only limits standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.Certificates, Licenses, Registrations:Valid Michigan Driver's license and good driving record.Physical Demands:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.
