Committed to our mission-driven approach, we believe in building meaningful relationships, embracing challenges as opportunities, and inspiring and being inspired every day.Position: Residential Medical CoordinatorSummary:Block Institute is seeking a dedicated Residential Medical Coordinator to work with our RN team to ensure quality medical care for individuals diagnosed with Intellectual and Developmental Disabilities. The Medical Coordinator will monitor healthcare processes in a residential setting and assist individuals in all areas of daily living.Essential Job Duties and Responsibilities:The duties and responsibilities of the Residential Medical Coordinator include, but are not limited to:Complete weekly inventory of all medication and ensure all medication is ordered and delivered.Oversee that the overnight AMAP is conducting weekly medication inventory checks.Check MAR daily for any missing signatures and report findings to RN and Management.Check all medical charting daily (Bowel Movements, Blood Pressure, Fingersticks).Ensure all controlled medications have a count sheet and that they are signed and counted daily.Check MAR monthly for current prescriptions and ensure a copy of the prescription is in the MAR book.Schedule all appointments in conjunction with the nurse.Prepare monthly MAR sheets, ensuring all information including medication names, dosages, and times are correct.Ensure all prescriptions have a drug-specific sheet to match in the MAR and that discontinued medications are documented and disposed of properly.Attend all medical meetings weekly or bi-weekly as per the residences schedule.Create and monitor tracking systems for Medication Storage, documentation, and training in conjunction with the RN.Track AMAP Certifications and Med Errors, reporting to the RN when an AMAP needs to be re-poured.Track all medical appointments, diagnostic, and lab testing in conjunction with the RN.Retrieve all lab reports or any other diagnostic testing results within two weeks.Ensure all incoming CVS forms are signed by the RN within a two-week period.Ensure MIPS forms are completed and attached to the CVS prior to appointments if needed.Ensure all CVS forms are filled out before any appointments with individuals information, including current medications and allergies.Ensure all AMAPs bring CVS forms to appointments and are aware of the type of appointment.Ensure detailed information is provided in the reason for visit section on CVS forms.Ensure adaptive equipment is on-site and in good repair following any recommendation.Ensure AMAPs are documenting all charting for individuals as required.Ensure monthly inventory of all OTC medications is checked for quantity and expiration date, disposing of expired medications immediately.In-service all new AMAP staff as suggested by the nurse.Attend and advocate at the majority of medical appointments, advocating on behalf of the individual to medical professionals and the IDT.Inform the nurse and management immediately of any medication changes.Contact the residence nurse after each medical appointment to inform them of the outcome.Oversee the AMAP communication book and ensure new entries are logged and read by all AMAPs.Perform DSP job responsibilities and any other duties deemed necessary for the safety and well-being of individuals we serve.All Block Employees will be required to learn and master the use of any and all software systems and technology utilized by the Agency, on a Program by Program basis.Qualifications:At least 2 years experience within an OPWDD residential settingValid Drivers LicenseHigh School Diploma or GED EquivalentAMAP Certification in good standingStrong Oral, Computer, and Written SkillsSCIP and CPR CertifiedBenefits:1199 Union MembershipMedical InsuranceDental InsuranceLife InsuranceVoluntary benefits such as critical care, disability, and additional life insurance403(b) Retirement AccountCommuter benefits planVacation and Sick Paid Time OffJoin Our Team!If you are passionate about making a difference in the lives of individuals with developmental disabilities and meet the qualifications, we encourage you to apply.Block Institute is an equal opportunity employer.
