We teach life skills and build healthy habits through skill-based groups and individual interactions. - Maintains physical environment and program standards to CHR, DMHAS, TJC, and DPH licensing standards.
- Establish rapport and working relationship, utilizing person centered practices.
- Effectively utilizes appropriate communication methods/tools to transfer information to other staff regarding the daily events.
- Teaches and assists all residents in areas of rehabilitation including nutrition and wellness, meal preparation, hygiene, laundry, banking and budgeting, basic home care skills, substance use education and recovery skills.
- Assists/Manages administrative functions and tasks at the direction of the supervisor.
- Prepares and facilitates educational and skill building groups and one to one intervention as directed by the treatment plan.
- Monitors medications of all residents in accordance with the Adult Residential Medication policy and procedure.
- Collaborates and coordinates care with providers and families as needed.
- Assists residents in addressing medical and psychiatric needs.
- Maintains documentation in compliance with agency and program standards.
- Participates in staff meetings, supervision, training, and other meetings as assigned.
- Additional duties as required.
QUALIFICATIONS:Education:Bachelor's degree required; a minimum of 1 year relevant experience preferred.Experience: Experience working with adults with Serious Mental Illness in a residential or supported living setting preferred; experience providing skills building teaching in group format preferred.Licensure/Certification/Registration: CPR/First-Aid Certification within 6 months of hire or according to program requirements.Valid driver's license required.Why Join CHR?
