DUTIES & RESPONSIBILITIES:

Work in a collaborative and supportive team to prepare individuals living with severe mental illness and substance use disorders who have a history of homelessness for more independent housing. Maintain a caseload of clients to work with, and develop an individualized plan to see clients based on an assessment of their needs and functioning. Use crisis management, motivational interviewing, and wellness modules to prepare clients for more independent housing. Host multiple wellness groups per week aimed at helping clients gain skills to improve self-sufficiency. Complete psychosocials for clients and participate in regular treatment planning for clients with their case managers. Coordinate all care and communication with psychiatric and treatment teams and advocate or the client as appropriate. Present clinical case reviews during team meetings and make suggestions for client care. Document all client interactions. This job is in a residential setting, so staff will be expected to meet with the client in person, and to also engage with the clients in the residence. Understand and comport to regulatory standards. Related duties as assigned.

HOURS:

Full-time, 37.5 hours per week

+ Monday - Friday; 10:00a - 6:30p

QUALIFICATIONS: MSW with provisional LMSW or LMSW license. Basic computer literacy required. CPR training certification or willingness to take training class in CPR/First Aid/Narcan.

*Vaccination preferred but not required

PROGRAM DESCRIPTION:

Cecil Ivory House is a transitional housing program targeting formerly homeless clients who are either mentally ill or dually diagnosed as mentally ill and chemically addicted. Funded and licensed by the NYS Office of Mental Health with additional funding from the US Department of Housing and Urban Development, Cecil Ivory House is located in Harlem and has a capacity of 24 beds. The mission of the program is to provide a safe and supportive environment where residents take part in rehabilitation interventions that will assist them in the attainment of their work, social and community living goals. A variety of services are available, including case management, substance abuse groups, medication monitoring, daily skills training and recreational activities.
