H. ASSIGNED DUTIES
DESCRIPTION OF WORK DUTIES AND RESPONSIBILITIES: (Type in or attach current

Duties Include:
Ability to perform leadership duties and responsibilities including, but not
limited to: Leading staff meetings, addressing conflict and others issues within
the team, organization, professionalism, consistent and healthy communication
among team members and with individuals served, review of 5% reviews monthly and
submit to Residential Coordinator and Deputy Director, review weekly case
management spreadsheets, Provide direct care services that includes skill
building and linkage to services, documentation that meets fidelity,
participation in agency, state and other trainings, participation on stakeholder
outreach and collaborative events. Other duties as assigned.
