Job Description:
We are currently looking for a Full Time Resource Specialist to join our Northern Community Health Resource team located at the Youth & Family Building working .Summary:To provide case management services to clients and determine eligibility through various federal and tribal programs that are administered through the Social Services division.Responsibilities:* Provides first class customer service - courteous and helpful to all internal and external customer types- Answers questions in a courteous and professional manner- Partners with internal and external services as needed to ensure appropriate referral is made- Greets and engages with a smile- Determines needs, seeks positive experience and outcomes* Completes all training and development requirements and participates in continuing education opportunities* Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types- Maintains general upkeep of work area and maintains a safe environment- Maintains and promotes professionalism- Punctual and consistent in meeting start time, work schedule, attendance and other assigned work time metrics- Adheres to Chickasaw Nation dress code standards* Maintains knowledge of and follows all laws, rules, regulations, policies and SOPs for assigned area, e.g. MICS, TICS, hospital standards, health codes, food sanitation, accounting principles, etc.* Reviews applications submitted to the program to verify submission of all needed information and documentation for accuracy* Provides current and potential applicants with information on eligibility, application procedures, certification and program process* Provides program information to vendors such as mortgage lenders and utility providers* Maintains confidentiality, security, and integrity of data and information in accordance with HIPAA regulations* Works on goals set forth by Counselor, Resource Manager or job developer* Prepares ad hoc reports* Provide holistic and comprehensive case management services to all clients* Contacts medical office to obtain necessary medical forms for the 477 Pathways to Success clients* Develop strategies to help clients improve their self-sufficiency* Provides recommendations/referrals according to clients need assessment for internal and external services* Supports and assists clients in all aspects of employment opportunities by demonstration, providing advice and coaching* Performs all other duties as assignedCompetencies:Essential:* Able to follow instructions, both written and oral * Able to adhere to all scheduling and attendance requirements* Maintains strict confidentiality in all matters* Good driving record and be insurable by the Chickasaw Nation* Must be able to work quickly, efficiently and accurately* Must be detail-oriented* Must be self-motivated* Ability to work independently* Honesty, truthfulness, reliability, accountability* Able to read, understand, apply, and retain knowledge of Chickasaw Nation, departmental, divisional and all other rules, regulations, policies, procedures and guidelines* Understand Native American culture and tribal programs* Must have empathetic understanding of common social problems (i.e., addictions, mental health problems, domestic violence, sexual assault, etc.)* Knowledge and understanding of First American populations* Ability to multitask* Establishes professional working relationships as appropriate* Works well within a team* Organizational and time-management* Ability to plan, organize and follow through on assignments* Able to verbally communicate in a professional and positive manner with other team members, departments, guests and management* Keeps written recordsSkills:Essential:* Basic Computer skillsExperience:Essential:* 3 years directly related experienceEducation:Nonessential:* Bachelors Degree in directly related fieldCredentials:Essential:* Pass general background check* Standard Driver's LicenseWorking Conditions:Essential:* Emergency* Fast-paced* Works in an office setting* Organized* Team-oriented* Communications phone, computer* Communications written, verbal* Communication with groups* May be required to de-escalate argumentative, hostile or similar situations* Interacts with young children* May be required to assist individuals with unmanaged emotions* High pressure* Interacts with clinical, support, technical, professional or management staff in a variety of settings and situations* Sometimes bend, kneel, stoop, push, reach* Use computer up to 75% of shift* Listening and hearing ability* Sometimes lift and or carry up to 25 lbs* Sitting up to 75% of shift* May work day shift* Home visits
