Communicates with medical staff, other departments, and outside agencies while maintaining confidentiality. Position requires self-motivation, creativity, and capabilities to function in a semi-autonomous role within a fast pace and dynamic environment. STANDARDS OF PERFORMANCE 1. Assumes direct resident care responsibilities as assigned by the Charge Nurse, to include, but not be limited to AM, PM care, backrubs, assisting with ambulation, assisting with dietary needs, dressing and undressing residents, and changing bed linen. 2. Carries out Special Orders or treatments as directed by the Charge Nurse, to include, but not be limited to special skin care, collecting specimens, intake and output measurements and documentation, vital signs, compresses, and enemas. 3. Ensures adherence to proper infection control, OSHA and safety standards. 4. Works closely with therapists and Restorative Nurse to establish restorative nursing goals and interventions for the resident's care plan. 5. Provides rehabilitative services to maintain and restore resident's maximum achievable self-care ability (i.e. ambulation, range of motion exercises, personal hygiene, toileting, feeding, transfers, positioning and strengthening exercises). 6. Provides encouragement to residents and family members. 7. Observes residents' condition and reports immediately, any untoward signs, symptoms or unusual occurrences. 8. Maintains current documentation of resident status on Activity of Daily Living Sheet and gives report to Charge Nurse. 9. Assists in Admission, transfer and discharge of residents. 10. Answers resident call-lights quickly whether that of assigned resident or not. 11. Escorts resident to other hospital services and/or departments as needed. 12. Accepts and completes Special Assignments in a timely manner. 13. Assists other staff in the completion of their duties. 14. Maintains certification in accordance with hospital, state, federal, and TJC regulations and guidelines, including 12 hours of resident related Inservice Education yearly to maintain state certification. 15. Maintains constant vigilance over residents, taking necessary safety precautions (rails up, call lights within reach, bed in lower position). 16. Maintains residents' room in a neat orderly manner with clear walk areas. 17. Assists residents to a car or other transport vehicle. 18. Transports residents, supplies, specimens, etc., as needed.19. Relieves in other areas as necessary. Requirements Minimum Level of Education: Education level equivalent to completion of High School diploma or documented C.N.A work experience will serve as equivalent to completed education. Formal Training: Completion of a Certified Nurse Aid Course of Study. Licensure, Certification, Registration: Must have and maintain a valid unencumbered Georgia Certified Nurse Aide, Restorative Aide, and current Basic Life Support CPR certification. Work Experience: One-year experience in healthcare setting as a CNA, Long-term care in a skilled nursing setting preferred.
