Job Description:
A healthcare organization in Western New York is currently seeking a licensed Registered Nurse (RN) to join their team in Erie County as a Hospice Admissions Nurse. In this role, the Hospice Admissions Nurse (RN) will be responsible for presentation of Hospice services to clients who have been referred for all levels of care (general inpatient, home care, and nursing home). Part-time and full-time opportunities are available! Responsibilities: The Hospice Admissions Nurse (RN) will:
  • Present hospice services to patients and families is individualized, sensitive to patient and families fears, needs and goals,
  • Perform comprehensive assessments to identify all physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness that must be addressed in order to promote the patient's well-being, comfort, and dignity throughout the dying process and documents findings appropriately in the EMR
  • Perform pain assessments using, adhering to pain protocols, knowledgeable in equi-analgesic dosing; optimal pain relief using an interdisciplinary focus and timely interventions
  • Articulate clinical course/disease progression of cancer(s), lung disease, heart disease, liver disease, dementia, neurologic disease states
  • Initiate the plan of care timely and appropriately with a demonstrated skill in anticipating problems that may cause or create a threat to patient safety or lack of symptom control
  • Perform other duties, as needed
Qualifications:
  • Graduate of an accredited School of Nursing
  • Current NYS Registered Nurse (RN) license
  • Solid assessment, clinical, and documentation skills
  • Working knowledge of Electronic Medical Records (EMRs)
  • Microsoft Office Suite proficient
  • Solid assessment, clinical, and documentation skills
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • 2+ years of Nursing experience
  • Bachelor's Degree in Nursing
  • Certification in Hospice and Palliative Care Nursing
