Full Time position includes FT ($5,000) Sign on ($1,250 paid quarterly) if hired and begins employment between July 1 and September 30, 2022

We are a Central Pennsylvania based Non-profit Hospice & Post-Acute Healthcare Organization specializing in home-centered care and we are seeking a talented Full Time RN Case Manager who will be responsible to provide nursing care in accordance with each patient's plan of care within our Cumberland County service area. This position will serve our valued clients wherever they may call home; inclusive of Long Term Care and Assisted Living/Personal Care Communities, Hospitals and in the patient's residence. This position works primarily a Monday through Friday schedule with weekend rotation.

Job Summary: Responsible to evaluate, establish and maintain a comprehensive plan of care that is individualized to specific patient circumstance and offers the patient/family direct clinical care and coordinates the care offered from all germane [interdisciplinary] team members.

Qualifications:

  • RN, with current PA license
  • Minimum of one year clinical nursing experience—hospice or home care experience helpful but not mandatory
  • Excellent communication skills [written, verbal, listening]
  • Ability to track and complete multiple tasks at one time
  • Ability to work independently; self-motivated
  • Ability to work as part of a team
  • Ability to work in a non-judgmental fashion with people of diverse backgrounds
  • Actively supports HCP philosophy of care and Key Performance Standards of Excellence
  • Ability to comprehend the stressors in one's own life in addition to the stressors inherent in a job focused on the end of life; ability to recognize/identify stress and to seek or accept help/assistance as needed
  • Proficient at basic computer/keyboard skills
  • Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing
  • Ability to independently access a patient's physical environment [e.g. negotiate steps]

Functions:

  • Possesses a knowledge regarding hospice regulations/reimbursement
  • Assess a patient/family's physical, psychosocial and spiritual needs with each visit
  • Establishes and maintains an individualized and up-to-date POC, in conjunction with the IDT and the primary physician, for each individual patient; POC incorporates direct care, teaching and counseling
  • Facilitates and coordinates nursing and/or physician orders for the patient
  • Inputs new patient orders, as well as all patient documentation, into software system in an accurate and timely manner as per agency policy
  • Orders supplies and equipment as needed for patient care
  • Infection control and safety measures/policies per agency policy/procedure
  • Supervises Hospice Aides per agency policy
  • Accepts calls from patient/family/physician and relays information as appropriate
  • Participates in continuing education and agency training as directed
  • Recognizes that quality improvement is a part of every staff member's job responsibilities
  • Identifies and reports areas for improvement and concerns for safety of patients, family members and staff
  • Stays abreast of performance improvement projects (PIPs) and their respective outcomes
  • Participates in on-call scheduling
