About the Department:

This department supports the collaboration efforts through patient and provider education, patient navigation, referring provider coordination, supporting consistent processes, and enabling strong and routine communication between East and West. This position would be employed by the CH-West Transplant Program but would be deployed at and embedded in the CH-East Royal Oak Max and Debra Ernst Heart Center. This individual would provide increased insight and enable increased connection to the East side providers and referring hospitals.

Scope of work

Responsible for providing quality education, follow-up and emotional support to the population and their families. Case manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings. Manages projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes.

  • Assesses the educational needs of clients/others. Develops and implements appropriate educational curricula/teaching strategies or makes appropriate referrals. Provides ongoing review and development of educational materials related to clinical management.
  • Case manages the population both in the inpatient and/or outpatient settings. Develops and manages clinical paths as appropriate, and measures outcomes of care to maximize continuity of care and appropriate resource utilization and length of stay. Coordinates multispecialty team clinics.
  • Provides direction to specified projects/programs pertaining to the population, assessing, planning, developing/maintaining, implementing and evaluating outcomes of same.
  • Documents educational process/patient information according to program/MDPH guidelines.
  • Communicates, collaborates, networks with and acts as a consultant to clients/significant others, other members of the health care team, and the community in order to ensure continuity of care and coordination of services, and to provide needed resources to the community.
  • Writes and reviews protocols, procedures and guidelines pertaining to clinical management.

Qualifications

  • Required Bachelor's Degree Nursing
  • Preferred Master's Degree Nursing
  • 5 years of relevant experience nursing experience required
  • Program development preferred
  • MI Registered Nurse (RN) Required
  • Basic Life Support (BLS) - AHA American Heart Association 90 Days required
  • Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association Solid Organ Transplant – Adult 1 Year required
  • Clinical Transplant Coordinator (CCTC) - ABTC American Board for Transplant Certification Solid Organ Transplant Upon Hire preferred
