A health services network in New York City is currently seeking a bilingual licensed Registered Nurse (RN) to join their staff as a UAS RN. In this field role, the Assessment Nurse will be responsible for doing UAS assessments in a tight area in Queens. Responsibilities: The Assessment Nurse (RN) will be responsible for:
- Performing home visits to conduct functional, psychosocial and environmental safety assessments
- Determining eligibility of enrollees
- Establishing an individual plan of care that enables them to remain safely at home despite long-term care needs
- Ensuring that DOH UAS information is complete and accurate
- Conducting follow-up assessments to ensure the members are receiving the proper care and services.
- Performing other duties, as needed
Qualifications:
- Prior UAS, MLTC or home care experience
- Bilingual in Cantonese or Mandarin
Desired Skills:
- Bachelor's Degree in Nursing
