In this hybrid role, you'll conduct in-home and remote assessments for individuals seeking community-based long-term care, empowering them to live independently in their own homes. Why Join Us? Key Responsibilities:Conduct in-depth assessments of individuals' health, functional abilities, strengths, and care needs.Review medical records and referrals to provide informed evaluations.Accurately input and code data in the UAS-NY (Uniform Assessment System of New York).Document all interactions, outcomes, and updates promptly.Stay up to date on policies and regulations affecting assessment quality.Collaborate with interdisciplinary teams to connect consumers with community resources.Engage with the Clinical Quality Assurance Department for case reviews.Support leadership and perform additional duties as needed. What You Need to Succeed: Education & Licensure:Graduate of an accredited nursing program (Bachelor's degree preferred).Active RN license in New York State.Valid New York State ID or Driver's License. Experience:At least 2 years in a home health setting within the last 10 years. (preferred not required)Familiarity with UAS assessments and experience working with geriatric or chronically ill populations is preferred. Skills:Strong interpersonal and communication abilities for engaging with diverse consumers.Proficiency with UAS criteria and New York nursing regulations.Ability to work independently in a fast-paced, deadline-driven environment.Empathy and cultural sensitivity when interacting with consumers. Other Requirements:Reliable transportation for travel to client homes within assigned regions.Ability to navigate technology, including telehealth platforms and Microsoft Office tools.
