Overview:UnityPoint-St Luke's HospitalPart-TimeWeekend Package20 hours per week; Friday, Saturday, SundayResponsible for the coordination, integration and facilitation of all care and services for the patient by all members of the health care team. Performs admission assessment, screening for appropriateness, and monitoring of plan of care to address physical and psychosocial needs, and provides problem solving assistance. Coordinates services, discharge planning, referrals to appropriate community agencies and assists with Advance Directives when appropriate. Responsible for reviewing and updating plan of care with healthcare team when applicable. Assesses, monitors, analyzes, and documents resources utilized in the provision of patient care. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.Qualifications:Minimum RequirementsIdentify items that are minimally required to perform the essential functions of this position.Preferred or SpecializedNot required to perform the essential functions of the position.Education:Registered Nurse· AD in Nursing, BSN preferred· CMAC (Certification as a Case Management Administrator)Experience:3 to 5 years clinical experience as an RN, with at least 1 year working with the diagnostic population to which the care coordinator will be assignedExperience in post-acute careLicense(s)/Certification(s):RN licensure in State of IowaValid driver's license when driving any vehicle for work-related reasons.Knowledge/Skills/Abilities:Excellent communication skills, particularly in face-to-face personal contact with patients and familiesCritical thinking and problem solving skills and coping effectively with stressful situations. Must possess time management skillsKnowledge of social services in a health care setting, including crisis intervention, support counseling and discharge planningUnderstand the concepts of levels of care, payment systems and insurance requirementsWorking knowledge of community information and resources for referralsKnowledge and skills necessary to provide services/care appropriate to the age and diagnosis of patients served across the continuum of care, in accordance with the hospital/departmental/unit standardsMaintain working knowledge of applicable Federal, State, and local laws and regulations as well as other policies and procedures in order to ensure adherence in a manner that reflects honest and professional behavior· Discretion in handling highly confidential information from patients and ability to work with highly diverse populationsOther:Use of usual and customary equipment used to perform essential functions of the position.
