Our Maternal-Fetal Medicine Centers provide innovative, comprehensive and interdisciplinary care for women. As the only Maternal-Fetal specialists in the region associated with an academic medical center, our specialists bring their own expertise as well as collaboration with neonatologists, geneticists and other University of Minnesota Health specialists to deliver the highest level of care for women and their babies - from preconception to post-partum.
We care for women who are of advanced maternal age, pregnant with a child who has indications of an abnormality such as spina bifida or Down Syndrome or pregnant with multiples. We also care for women who have a history of preterm delivery or labor, an incompetent cervix, gestational diabetes, history of preeclampsia, pre-existing medical conditions, such as cardiac disease or cystic fibrosis, prior fetal or neonatal death and recurrent pregnancy losses.
This position will be working on a casual/as needed basis on the day shift, 8 hour shifts, no weekends. This position is not eligible for benefits.
Responsibilities/Job Description
The Maternal-Fetal RN Coordinator is responsible for the triage and assessment of referrals received from outside medical centers, offices, and internal referrals for MFM consult, fetal diagnosis and treatment center (FDTC) and complete obstetric care. Coordinates and provides direct patient care for all Fetal Diagnosis and Treatment Center and Perinatal Assessment and Consultation Clinic patients ensuring communication with internal perinatal services care teams, referring providers and pediatric specialists. Performs the daily operations along with physician leaders of the program.
* Provides direct patient care for perinatal assessment and consultative clinic (PAC) and fetal diagnosis and treatment center (FDTC) patients
* Completes external fetal monitoring
* Arranges, facilitates, and follows up on multidisciplinary maternal or fetal care conferences
* Facilitates biweekly multidisciplinary MFM complete care patient call
* Attends monthly interdisciplinary meetings: FDTC and Cardio-Obstetrics, contributing pertinent patient information and follows up on provider recommendation and contributes to the development and implementation of individualized patient care plans.
* Manages system maternal hypertension patient follow-up as directed by MFM provider(s)
* Acts as one point of contact for patients, physicians, and care providers throughout the episode of care in the perinatal assessment and consultation clinic and fetal diagnosis and treatment center in MFM.
* Reviews all referrals for MFM consultation, reviewing pertinent records, obtaining necessary records, reviewing history with patient prior to presenting case to the MFM provider for timing of consultation related to pregnancy or preconception indication.
* manages program planning, coordination, and quality monitoring to ensure that families who opt for perinatal palliative care preserve hope while preparing for the possibility or certainty of the death of their baby
* Provides care, education, and support for patients during pregnancy terminations.
Qualifications
Required
Education
* Bachelor's Degree in Nursing
Experience
* 5 years clinical experience (2 of which are required to be labor and delivery)
License/Certification/Registration
* Active MN Registered Nurse license
* Basic Life Support (BLS)
Preferred
Experience
* 1+ years working as a care coordinator/case manager
License/Certification/Registration
* Case Management Certification
