Case Manager - Wilson Medical Center Responsible for facilitating internal and external delivery of healthcare services, integrating utilization management, case management, and discharge planning to promote a collaborative interdisciplinary care delivery model and achieve optimal clinical and financial outcomes. Key responsibilities include: Performing initial and ongoing discharge screens/evaluations to determine general health behaviors, practices, and resources that will influence the patient and family's abilities to manage chronic conditions optimally. Collaborating with the healthcare team to determine the extent of functional deficits and areas for potential improvement. Assessing emotional status and quality of relationships and adequacy of social support systems, identifying new sources of support. Prioritizing immediate plan for barriers to discharge, including medication availability, shelter, and primary care physician coordinating care resources accordingly. Ensuring patient/family/caregiver understands and has access to all prescribed post-discharge therapies. Communicating with patients/families from admission to discharge. Performing clinical reviews based on the Utilization Review Plan with follow-up actions identified for medical necessity and level of care. Maintaining clinical and financial documentation. Participating in performance improvement processes and other duties as required by the job.
