Behavioral Health RN Case Manager - Full-Time, Salaried
Schedule: Monday to Friday, 8:00 AM - 5:00 PM

Key Responsibilities:
* Patient Care Management: Oversee and coordinate a caseload of behavioral health patients, assessing their physical, psychosocial, and environmental needs.
* Plan of Care Development: Create and implement individualized treatment plans, adjusting them as necessary.
* Collaboration: Communicate significant findings to the Clinical Supervisor, physician, and other healthcare professionals to ensure optimal patient care.
* Education & Support: Educate patients, families, and care staff on behavioral health best practices, symptom management, and follow-up care.
* Supervision: Lead and supervise behavioral health aides and LPNs to ensure top-tier care delivery.
What You'll Need:
* Current, unrestricted state RN licensee
* 1+ years of experience in a clinical care setting as a nurse
* Able and willing to travel within branch/office coverage area.
* Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
* Must be able and willing to travel 50%
* Associates Degree or Bachelors Degree in Nursing is preferred
* Previous Home Health or Hospice Experience is preferred
* Previous experience with Homecare HomeBase is preferred
