Job DescriptionDescriptionThe RN (Registered Nurse) Case Manager provides case management and nursing services to Hudson Valley Hospice patients and their families and to ensure that those services are consistent with the philosophy, goals, objectives and policies of the Agency. Register Nurse (RN) Case Managers work in various settings: Nursing Homes, Home Care and In-Patient (throughout various hospitals). As an RN Case Manager, you are assigned to a specific team to work within; Home Care, Nursing Home or In-Patient. Each RN Case Manager provides pain and symptom management, recommendations and collaborates with the other Interdisciplinary Team members.Essential Functions
  • Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural and spiritual assessments to ensure and coordinate the delivery of comprehensive, holistic services by the Interdisciplinary Hospice Team.
  • Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care.
  • Documents and enters into EMR all interventions of phone and home visits within 24 hours of the intervention. Medication changes will be entered by the end of the shift.
  • Develop and prioritize the interdisciplinary care plan goals of the patient/family, hospice team and attending physician, prior to team meetings.
  • Update the care plan a minimum of every 14 days.
  • Document and computer entry of all interventions of phone and home visits within 24 hours of the intervention.
  • Provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur.
  • Teach, supervise and counsel patients/families/care givers regarding physical care, observing return demonstration of procedures as appropriate.
  • Effectively communicates patient status to family members and or caregivers.
  • Revises nursing diagnosis, goals and interventions, expected outcomes, and plan of care based on assessment of data and input of patients and family members prior to the IDG meeting.
  • Provide Coordinator or DPS and On Call staff report on all changes in patient/family condition and POC.
  • Follows and upholds all state and federal laws and regulations outlined in the policy and procedure manuals.

  • Can organize appropriately with efficient time management skills
Education & Experience
  • Graduate of an accredited nursing school.
  • Valid New York State nursing license.
  • Two years nursing experience required
  • One year medical-surgery experience highly preferred

  • Work in various home environments, nursing homes or hospitals that Hudson Valley Hospice is contracted with
  • Moderate/High level of possible exposure to infectious agents.
  • Moderate amount of travel by car
  • Moderate to high risk of exposure to unsafe environments.
  • Routine duties involve potential for direct exposure to blood or body fluids.
  • Valid New York State drivers license with reliable transportation.
