About the Role: We are currently seeking a Hospice RN Case Manager for our esteemed client in Cleveland, OH. This is a direct placement opportunity, offering you the chance to step into a key role with a leading regional healthcare provider. In this vital position, youll focus on delivering compassionate care and ensuring the highest standards in end-of-life care for patients and their families. What Youll Do:
- Provide nursing services in accordance with the states Nurse Practice Act, facility policies, and Medicare guidelines.
- Review client referrals and respond appropriately, collaborating with clinical supervisors to determine eligibility for Hospice services.
- Conduct comprehensive assessments, including OASIS data, within 24 hours of referral, unless other arrangements are made.
- Document findings, plans, and client progress in accordance with agency guidelines.
- Develop and coordinate a comprehensive plan of care, collaborating with physicians, therapists, social services, and other healthcare professionals.
- Provide on-call support and collaborate with a multidisciplinary team to ensure effective Hospice services.
- Ensure a safe environment for clients and coworkers, adhering to infection control practices and safety procedures.
Your Background:
- Education: Graduate of an approved school of professional nursing. A Baccalaureate degree is preferred.
- Experience: Minimum of 2 years of nursing experience, with Hospice experience preferred.
- Licensure: Current RN license in the state of Ohio. Valid drivers license with an insured automobile in good repair.
- Skills: Strong communication, organizational, and time management skills. Knowledge of home health regulatory and reimbursement requirements.
