Job Description:
RN Case Manager The RN case manager serves as a primary point of contact to patients and caregivers throughout their journey with our practice. The RN case manager will be expected to provide patient support and advocacy for patients with chronic illness. They will be involved in all aspects of patients' journey from onboarding, care plan development, medical coordination, and long-term treatment adherence. RESPONSIBILITIES: Assigned tasks would include but are not limited to the following:
  • Relationship building with patients, collaborating between the patient and community providers.
  • Ensure patients treatment compliance and provide resolutions to treatment issues through communication, education, and collaboration.
  • Perform follow up tasks assigned after virtual medical visits provided by Nurse Practitioners.
  • Follow up items may include assistance with referral processing, communication with referral sources, primary care and specialty providers, sending vital signs or other reports to providers, arrange for prescriptions or laboratory services.
  • Coordinate communications across various providers and patients/families by connecting patients with social and community support systems for the continuation of care.
  • Lead Chronic Care Management and Remote Patient Monitoring initiatives as the company expands.
  • Oversee clinical training
  • Assists in new hire onboarding procedures
  • Chart audit/review
  • Development of quality improvement initiatives
  • Tracking and reporting of quality assurance measures resulting in improved patient outcomes and the promotion of exceptional patient care.
  • Healthcare compliance following industry guidelines
QUALIFICATIONS:
  • Unrestricted RN License in one or more of the following states: CT, NY, NJ, MA, ME, NH, PA, required
  • 2+ years of experience as an RN, Case Manager or a combination of relevant education and experience required
  • Excellent interpersonal skills
  • Ability to always communicate clearly and effectively with patients and other external parties in a courteous and friendly manner
  • Must be detail-oriented and highly organized
  • Firm grasp on medical practices, administrative processes, and organizational policies
  • Knowledge of patient care and examination procedures
  • Technical proficiency including basic word and excel skills, ability to learn various electronic documentation platforms
  • Must maintain patient confidentiality at all times
PAY RANGE: $85,000-$90,000 based on experience
