Job Description40 hrs/wk, 8a-4:30p. every other weekend and every other holiday The Case Manager is responsible for providing direction and leadership for patient progression and care coordination. In collaboration with other internal and external disciplines, the Case Manager directs and coordinates the care of patients from admission to transfer or discharge with the goal of cost-effective utilization of hospital resources, minimizing out of pocket expenses for patients, providing appropriate utilization of resources, and patient advocacy. This position reports to the Director, Case Management and Social Work and operates within established organizational and departmental policies and procedures. REQUIREMENTS: Graduate of an approved, accredited school of professional nursing required; BSN preferred Licensure in the Commonwealth of Massachusetts as Registered Nurse required Case Management certification or active pursuit of certification preferred One to two years of related clinical, continuing care, or case management experience preferred Excellent written and oral communication skills required EPIC experience required
