This philosophy extends not only to our clients, but to our staff as well.As the RN Case Manager, you will be responsible for developing and maintaining client-specific care plans, ensuring these plans are followed, and providing for the client's ongoing needs in a private pay, concierge level home care. This role requires 70% fieldwork for a small caseload of clients demanding great attention to detail.The salary for this full-time position ranges from $110,000 to $130,000, commensurate with related experience.QUALIFICATIONS:Current NYS RN license in good standingBSN preferredMinimum of two years recent clinical experience within a Home Care settingStrong assessment and organizational skillsStrong time management skillsPrior supervisory experience with field staffProven track record of working independentlyPRIMARY RESPONSIBILITIES:Ensuring the highest quality of client careDeveloping care plansObtaining necessary physician orders for home care cases as required by State regulationsReporting changes in the client's medical or mental condition to the attending physician and the Agency's team involved in the client's careSupervising and supporting the home care teamStriving to improve the lives and sustainability of our clients by supporting their autonomy in the home environmentBENEFITS:Flexible schedulePaid time offReferral programUnlimited Monthly Metro CardComprehensive medical, dental and vision insurance401K with company matching option
