*Ghost RX* is searching for *RN Case Managers* to work at the *Indian Health Services facility* located in *Santa Fe, NM*. Any state license is accepted.*Work Shift*: 8 Hours - (8:00 a.m. - 4:30 p.m.) with 30- minute non-paid lunch. The contractor must be assigned hours bi-weekly; scheduled for weekends and Federal holidays; no on call; as scheduled by the COR in writing based on the facilities need.*Duties** Conduct comprehensive assessments of patients to develop individualized care plans.* Coordinate with healthcare providers to ensure timely diagnostic evaluations and treatment.* Manage medical records and documentation to maintain accurate patient information.* Facilitate communication between patients, families, and the healthcare team to support care decisions.* Monitor patient progress and adjust care plans as necessary based on ongoing evaluations.* Receives and directs telephone calls to appropriate parties utilizing proper telephone etiquette and basic interpersonal relationships to work with a variety of individuals.* Collaborate with interdisciplinary teams to optimize patient outcomes in both clinic and ER settings.* Participate in utilization management activities to ensure appropriate use of healthcare resources.* Contractor must have knowledge of the IHS (RCIS, RPMS, EHR, iCARE) computerized clinical and pertinent administrative information systems.
